The former Manchester United,Leicester City and Torquay United manager died at the age of 94.
After 18 months, Sir Matt Busby decided to replace him with another man.
He was capped nine times by the Republic of Ireland.
In a statement, United said they were deeply sad to learn of O' Farrell's passing.
There is no doubt that Frank was a talented manager and a man of integrity, according to the Old United club.
The first of three spells led to the Division Four title in 1966.
He succeeded Busby in 1971 and led them to the 1971 Division Two title and the 1969 FA Cup final.
In his first season in charge, Manchester United finished eighth, but he was fired in December 1972 after the club finished third from the bottom of Division One.
He was the manager of the Iran national team and later the manager of Al-Shaab in the United Arab Emirates.
Having achieved so much at the club, Frank O' Farrell is considered to be a club legend.
Although his passing leaves a void that will never be filled, we remain privileged that such a large and successful part of his career was spent at our club.
O' Farrell was the oldest living former West Ham player prior to his passing on Sunday morning at the age of 94 years and five months.
Everyone at West Ham United would like to express their sincere sympathies to Frank's family and friends.
