The title favourite saved a match point before coming back to beat Felix Auger-Aliassime in the Australian Open quarter-finals.
The first player to fight back from two sets down in this year's Australian Open was second seed Medvedev.
The fourth seed, Tsitsipas, defeated Jannik Sinner in straight sets.
The young man was beaten by the young man in last year's semis.
The chance to avenge that defeat against the Russian and reach his first Australian Open final has been given to him.
The third seed from Greece defeated the 11th seed from Italy, Sinner, to reach the semi-finals for the third time.
Sinner failed to get a break point.
The opening set was wrapped up in 36 minutes and Tsitsipas broke early in the second to take a 2-0 lead.
The court had to be towelled down because the roof needed to come on. After an 18-minute break, the players came back and Tsitsipas closed out the set.
Matteo Berrettini is the first Italian man to reach the Australian Open semi-finals.
The Greek broke early in the third to take a 2-0 lead and he was unable to get a glove on the serve.
After breaking again, he clinically polished off the victory to progress in two hours six minutes and continue his pursuit of a first Grand Slam.
"I am very, very happy with the way I served today and used my tactics, and I was also pleased with how I adapted to the roof closing," he said.
It is part of the game. You don't know what to expect from the weather. I knew I was going in the right direction.
The conditions might have changed a little bit, but my mind kept going. I tried to adapt and it worked.
