According to the document, the 94-year-old Hollywood icon died at home in Beverly Hills. His occupation is listed as an actor with 76 years of experience.

He had some of the best roles in the '60s, including "To Sir, With Love," "In the Heat of the Night" and "Guess Who's Coming to Dinner."
