Reece James also received the decisive penalty during the previous round
Chelsea reached the quarter-finals in the Carabao Cup after beating Southampton, Premier League rivals, on penalties following a 1-1 draw.
Reece James scored Reece's decisive spot-kick to make the league leaders, after Will Smallbone and Theo Walcott failed to score with their efforts for Saints.
Both sides made the changes to tie the game. Chelsea brought six players from their 7-0 win over Norwich in the Premier League, while the visitors had nine.
The hosts settled down and took the lead just before the half-time break when Kai Havertz scored his third goal of this season from a corner.
Southampton responded immediately after the restart, when Che Adams shot in from close range following Kepa's mishandling of Kyle Walker-Peters’ shot.
As the goal scored, the game came to life. The ball was then quickly moved from one end to another.
Fraser Forster, who tipped over a James driver after a short free kick, prevented the hosts regaining the lead. Substitute Shane Long was wide for the visitors.
Smallbone was close to marking his return to the team after nine months away following an ACL injury. However, his final header was saved from Kepa who made two spectacular stops in stoppage to deny Lyanco, and Mohammed Salisu.
These were crucial because Chelsea won the penalty shootout and remained in contention for silverware.
The quarter-final draw will take place on Saturday, at 10:30 BST.
High-flying Blues do the right thing
Chelsea had been ahead by half-time thanks to Kai Havertz
The Blues are enjoying a great start to the campaign at home and abroad. They currently hold the Premier League lead and have won two of three Champions League games.
Thomas Tuchel, however, was faced with a lot of games and decided to rest some of his first-teamers and give those on the fringes an opportunity to shine.
Mixed performances were displayed by those who came in. Kepa stood out with his critical double save, while Saul Niguez twice got close to opening his club's goal account.
Ross Barkley, however - who was making his first season start - and Hakim Zyech were left largely unidentified and took off with 23 minutes remaining.
After beating Aston Villa 4-3, Chelsea has now needed penalties for the League Cup. But they won the game.
This will be a valuable opportunity for Tuchel to get to know his players, as the Blues fight for success in many competitions.
