The exclusion of the goalkeeper from the team was a big shock.
When healthy, Steffen had been locked in a battle with Matt Turner for the starting spot, with Berhalter usually picking Steffen for the bigger matches.
He played in 16 of the club's 20 matches in England's second-tier Championship in order to get more first-team minutes.
It wasn't enough for Turner, Horvath and Johnson to make the roster.
Injuries played a role in the makeup of the World Cup roster, as every World Cup roster has its share of near misses. Atlanta United's Miles Robinson and Crystal Palace's Chris Richards didn't make it because of injuries.
The other players healed up in time to play in this weekend's final round of club games.

Tim Ream was one of the biggest surprises.
In September of 2021, Ream played in the first World Cup qualification fixture. The injuries to Robinson and Richards made it possible for him to be selected.
None of the candidates really staked their claim to a starting spot in the center forward position.
It was a bit of a surprise that Haji Wright was included at the expense of a player from the other team.
Both players had been in excellent form of late for their clubs, with Wright scoring nine goals on the season and four in his past four games, whilePepi had five goals and two assists since moving to the Dutch side on loan in September
Wright was one of the three people who got the nod.
With Sergino Dest dealing with an adductor issue, Berhalter decided to use additional cover at right back and chose Moore over Reggie Cannon.
At the World Cup, the US will have an average age of 25 years, 175 days, making it the second- youngest team in the tournament. The 1990 squad was the youngest.
After the U.S. failed to qualify for the World Cup, only one player with previous World Cup experience has been selected.
The U.S. will play England and Iran in Group B.
There will be a full U.S. roster for the World Cup.
